Clearances & Required Paperwork
Background check requirements for school employees, contractors and volunteers can be returned to:

If you are offered a position with the Littlestown Area School District, please be advised that the following information will be required before you are able to begin working:
State Police Clearance (Act 34) - complete online and print your results immediately
Child Abuse Clearance (Act 151) - effective 12/31/14, submit electronically
FBI Fingerprints (Act 114) EMPLOYMENT ONLY - register with service code 1KG6XN then go to the fingerprint location to be printed
Once you have your appointment scheduled, contact Vanessa Baker bakerv@lasd.k12.pa.us to provide your UEID# that is provided on your appointment confirmation. The district will pull your official results on your behalf.
School Health Record - includes physical & TB test from within a year. TB test results must be within 60 days from the date it was administered.
Act 24 (PDE Form 6004) - Arrest/Conviction Report
Verification of Employment Form - for Teachers ONLY to complete the top portion of the form and send to all previous employer(s) to verify your step for the salary scale.
PA Sexual Misconduct/Abuse Disclosure Release (Act 168) - A separate form must be completed for:
Current employer
All former employers that were school entities; and
All former employers where you were in a position that involved direct contact with children.
